About Us

The Sheffield Wheelchair & Special Seating Service covers a wide catchment area, including Sheffield, Barnsley and Bassetlaw. It is one of the largest adult and paediatric services of its kind in the country. The clinical team, which consists of technicians, engineers and therapists, see thousands of patients each year in a mixture of settings. Many patients are invited to attend for an appointment at the Centre itself, whilst others are seen in their own homes, day centres or schools.

Making a Referral

We accept referrals from GPs, therapists and community nurses. To submit a referral, please follow the process below. We do not accept paper, fax or telephone referrals.

To refer a patient there are 3 steps:

Step 1: Obtain a password for the referral form by calling 0114 2266833.

Step 2: Download and save the referral form below to your PC as an Adobe Acrobat document.

Step 3: Open the Adobe Acrobat file, complete all the fields in the document and click ‘submit’. If you would like to check your referral has been sent correctly please check your sent email items. An email will have been sent with your attached referral to sth.wheelchairreferral@nhs.net

Please note: if the ‘submit’ button fails to work, please save the completed referral, and attach it to an email which can be sent to sth.wheelchairreferral@nhs.net

If you submit an old form, or an incomplete data set, we will return your referral to you and ask you to transfer the referral to the latest form version and complete any missing data items. We need to take this step to be able to process your referral in a timely manner.

Once your referral has been received we will contact you to confirm the next steps. It is crucially important that the contact details for the referring healthcare professional are up-to-date.
