Referral Criteria
- Mainly older adults. Some younger patients with Stroke or Parkinson's disease who have multidisciplinary needs.
- Patients must have the potential for rehabilitation.
- Patients must have the cognitive ability to follow a programme of rehabilitation.
- Patients must have multidisciplinary needs.
- Patients requiring transport must live in the south of the city.
- Funding must be provided for patients who have non Sheffield GPs.
- Referrals can be made by any Health or Social Care professional
Referral process
Referrals can be made by any Health or Social Care professional using the following:-
- Contact Single Assessment Process (SAP) and ARC referral form.
or
- Contact SAP with accompanying letter.
Referrals can be posted or faxed.
Telephone enquiries welcome.
Patients or carers known to ARC can contact by phone or letter to make a self referral.
