Information for health professionals - Active Programmes (Cardiac and Pulmonary Rehabilitation)

This service includes Pulmonary Rehabilitation and the Cardiac pathway from hospital discharge to Cardiac Rehabilitation

Pulmonary Rehabilitation

Pulmonary rehabilitation is a combined exercise and education programme for people with chronic lung disease such as COPD, ILD and Bronchiectasis.

Exercise is individually tailored and patients are progressed in accordance with their fitness levels and breathlessness. Pulmonary rehabilitation groups are provided across Sheffield at various venues.

We can also offer a shorter home pulmonary rehabilitation programme for people that cannot get to group.
The pulmonary service also has an Early home rehab programme providing support for people who are recovering from an exacerbation in hospital – currently accepting referral from COPD specialist nurses only.

Referrals accepted via Single Point of Access (SPA). Referral forms are on the STH intranet site, Community Services, A-Z of services, Active Programmes, Referrals.

Cardiac post hospital discharge pathway

This phase of cardiac rehabilitation occurs when a patient has been discharged from hospital. This early post discharge period is when many patients feel anxious and sometimes isolated.This supportive phase has been shown to reduce anxiety and depression and reduce the number of hospital re-admission rates.The service contacts all patients upon receipt of referral from secondary care. Following a triage process appropriate home visits are booked, usually within 5 days of a patient being discharged home.

The team aim to:

  • Prevent onset of further cardiac events
  • Prevent unplanned re-admissions to hospital
  • Reduce length of stay in hospital
  • Enhance quality of life
  • Promote self management skills

Cardiac Rehabilitation

Cardiac rehabilitation is an essential component of the treatment and secondary prevention of coronary heart disease. People with cardiac disease, in partnership with a multi-disciplinary team of health professionals, are encouraged and supported to achieve and maintain optimal physical and psychological health. The goals of cardiac rehabilitation (CR) are to help patients achieve the best possible quality of life and functional level and to reduce mortality and morbidity through secondary prevention. Cardiac Rehabilitation groups are provided across Sheffield.
