Active Programmes (Cardiac and Pulmonary Rehabilitation)

The Active Programmes Team provides support and rehabilitation to people with cardiac and chronic respiratory conditions.

If you have been in hospital with a heart attack or for cardiac surgery you will be offered a referral to the Community Cardiac Rehabilitation Team. The Community Cardiac Rehabilitation Team will contact you at home and offer you support through a self-help manual or inviting you to a group cardiac rehabilitation programme.

Rehabilitation can also be offered to people with heart failure or atrial fibrillation.

The cardiac team also can provide a home visit for patients who are newly diagnosed with Angina to offer advice so they can manage their condition better.

Pulmonary Rehabilitation is a specially designed programme for people with chronic lung disease. It is a combination of exercise and education that will help you become more control of your breathing and active. This can be in a group or at home depending on your circumstances.

Your can be referred by your GP, hospital consultant or any other health care professional.
