Patients with left ventricular systolic dysfunction (LVSD) are referred to the Specialist Heart Failure Nursing Service (SHFS) when discharged from Sheffield Teaching Hospital.
Referrals are also received from GPs for patients who have symptomatic heart failure and are at risk of a hospital admission.

At risk of hospital admission for worsening heart failure (without a recent admission for Left ventricular systolic dysfunction (LVSD)
Meets case management criteria (levels2 or 3)
Referral process
STHFT wards refer to SHFS in secondary care via the SHFS office (designated phone and fax), onward referral to community team via SPA
Specialist nurses assess patients against criteria and prioritise according to urgency.
GP’s refer through the Single Point of Access (SPA).
