Individuals are usually seen in their own homes.
Operating Hours 08.00 - 17.00 Monday to Friday
Referral Criteria The referred individual must have a long term respiratory condition (eg COPD, pulmonary fibrosis, asthma) and has an identified co-existing mental health need.
Is over the age of 18
Ideally have completed a GAD7 and PHQ9 within the last 4 weeks
The referred individual must be registered with a Sheffield GP
The referred Individual is not actively receiving a service from an IAPT or Community Mental Health worker (although they can currently be under a secondary care mental health consultant).
