Patient Portal UK (PPUK)
Patient Portal UK (PPUK) is a digital platform used by some of our services that allows you to complete pre-assessment questionnaires and view the upcoming appointments they are linked to.
Sometimes, we may need to collect information from you about your health before an appointment. If you are able to use a computer or mobile phone, we can gather much of the information that we need by an electronic health questionnaire. This is sent securely via the patient portal. Filling in a questionnaire means the information is available to the clinician in our electronic patient record ready for your appointment.
When a service that is using PPUK schedules an appointment that requires the completion of a questionnaire, we will automatically send you a text message inviting you to join the patient portal. If you have previously registered and used the portal you will receive the questionnaire in the portal.

When you receive the message, click on the secure link. Alternatively, you can register from your computer, tablet or smartphone at https://patients.sth.nhs.uk/
You will be taken to a page that looks like this. Follow the instructions as prompted.

To register, you will need to be currently under Sheffield Teaching Hospitals care, aged 16+, with an NHS number and a valid mobile phone number on our records.
Each questionnaire sent will be linked to a specific appointment and the date and time. For example:

The portal will send a reminder to complete the questionnaire seven days and two days prior to the appointment.
Can I choose to opt out?
Registering is optional and if you do not sign up, we will continue to communicate with you by post and phone. If you choose not to register now, you can change your mind at any time before future appointments.
