Register/Login

There are a number of different ways you can register for MyPathway.

Once you become a patient under the care of Sheffield Teaching Hospitals NHS Foundatipn Trust you will receive an invite to join MyPathway via email, text or letter. This invite is unique to you and will contain all the information you need to register. 

Alternatively, you can sign up here, so please ensure you have your NHS number to hand.

Once you have joined MyPathway you will start to receive: 

• Appointment information including locations 
• Questionnaires to fill in about different aspects of your health and wellbeing relating to your appointment 
• Information that we think might be relevant to you, or that you request about your condition, care or appointment
