The meeting is free and being held virtually on MS Teams so it is easy to join.
We invite you to hear about how your local adult hospitals and community health services have performed over the past year, some of the fantastic work that has been going on and our plans for the future.
This year's meeting also includes a presentation from one of our clinical teams on how we are using data to adapt diabetic care to reduce inequalities.
There will the opportunity to ask questions, but please note that we cannot answer questions about individual patient care - please send any questions ahead of the meeting to jane.pellegrina1@nhs.net
