Referrals (Haematology)

GPs

Please use the NHS e-Referrals service to refer a patient to our Haematology service.

Secondary care

Clinical Haematology secondary and tertiary referrals can be sent by email to:

For urgent advice, please contact the department on 0114 271 1900 and ask for the on-call specialist haematology registrar.

Referrals to the late effects service will need to be made via your GP or another Healthcare Professional.

The following information is required in the referral letter:

  • Primary cancer diagnosis and date
  • Cancer treatment details (for example surgery, chemotherapy, other systemic cancer therapy and radiotherapy) with dates. If not treated in Sheffield, please provide name of details of the cancer centre/hospital where treated was received
  • Current problem related to previous cancer and cancer treatment.
    Relevant previous medical and social history
  • Details of current medications

Please note, people experiencing chronic pain problems, including cancer treatment induced peripheral neuropathy, need to be referred directly to an appropriate local pain service.
