Our AKI workstream has established connections with the AKI workforce across the region to gather baseline information on regional unit activities and data on transfer delays (in Hull and Bradford in particular). We have also worked to establish connections with CCNs which has assisted us in the development of an updated Regional AKI Transfer Guideline which is ready to pilot in Leeds and Calderdale before being rolled out across the region.
We have started a data sharing practice to identify areas for change and encourage the sharing of ideas such as making AKI a mandatory Trust audit (done in Leeds), GP Assist primary care pathway development (Bradford) and hydration tools within electronic record (Calderdale). Moreover, promoting Kidney Health in the community is another significant aim of our network, therefore, we are building links with ICB data analysts to identify who to target in the community and have been building connections with primary care services and educational providers for GP trainees across this region as well as discuss the development of a Region wide AKI patient information leaflet.
