Renal Young Adults Team

Who are we?

Our team helps young patients who need to make a change from children’s services to our adult services and those young adults coming to a renal unit for the first time. We aim to make sure the change go as well as possible for you, and the people that look after you.

The transitions team will try to help and support you in many of different ways. These may include:

An informal tour of Sheffield Kidney Institute before your first clinic appointment. This may not always be possible, for example, if you come here in an emergency. But can be arranged afterwards.
A named member of the transitions team to welcome you, and to help you settle into adult services. They will keep working with you to help you to settle in, for as long as you feel you need their help.
You may also be introduced to one of our patients who has already settled in here in adult services. They can talk to you about the unit, and maybe answer some of your questions about adult services. Please ask if you think this may help you.

The move from children’s services to adult services is a very important time for you and your family. The Young Adults team is here to make this change go as well as possible for you and the people who look after you.

We will carry on offering you support for as long as you need it in order to settle in to adult services.
