The CKD workstream continues to collaborate with multiple stakeholders including ICBs and primary care providers within the region and with senior leadership at the long-term condition board. One of the key aims we are focussing on is to increase CKD awareness via providing education and training for not only renal staff but also primary care providers to address health inequality and develop strategies to educate and engage with individuals at risk. A recent Kidney Care UK report noted 65% of people with diabetes and hypertension (who went on to be diagnosed with CKD) were not made aware of their kidney risk before diagnosis, therefore, whilst this is a challenging project this will lead to positive impact in patient care with more medical optimisation, prevention to progression of the CKD and less late presentations to the renal services with likely more pre-emptive transplantation and planned start to dialysis.
