A city wide service to adults 18+ requiring input in their own homes, including residential and nursing homes. We provide SLT assessment and intervention for people with swallowing and/or communication disorders. Clients will often have multi pathology, including dementia. The majority of our clients are over 65 years of age. Intervention includes input aimed to help carers support the client. Whilst the team is uni-disciplinary it does link with other services particularly GPs, district nurses, community matrons, the Integrated Care Therapy Team, community dieticians and SLTs in other teams.
Adults Requiring a Domiciliary or Care Home SLT Service
We aim to see urgent referrals within 2 weeks and routine referrals within 4 weeks.

Referrals can be made by any health or social care professional or by the client/carer directly. SLT referrals should be made using the Adult Referral Form and e-mailed to: sth.sltreferrals@nhs.net. Please call the office about encryption if you do not have a secure email address. This referral form is also available to GPs on the online Press Portal.
