Palliative Care Support at Home

Formerly known as The Intensive Home Nursing Service.

Palliative Care Support at Home (PCSH) is a city wide service providing short term care to patients who are approaching the End Of Life and wish to stay at home. This care will be provided in the patient’s own home by Clinical Support Workers.

We provide a responsive seven-day service to patients registered with a Sheffield GP 365 days a year. Our service works closely with the Community Nursing Team, St Luke's Hospice and other Health Care Professionals.

The service provides varying levels of care depending on the needs of the individual, which may include:

  • Visiting Support, to provide personal care for up to 4 visits a day.
  • Night Care Support from 10pm to 7am.
  • 24 hour care when a patient reaches their last days to hours of their life.
  • Carer Support, which provides carers respite from their caring responsibilities for up to 6 hours each week for a period of 8 weeks.

To access this service you will need a referral from your GP or other healthcare professional involved in your care.

Patients referred to this service should have an approved CHC fast track and be registered with a Sheffield GP and Community Nursing team.
