Intensive home nursing

The Intensive home nursing service is a service which provides care for patients who are approaching the last days to weeks of their life and wish to die in their own residence. We provide a responsive 7-day and night service to patients registered with a Sheffield GP 365 day a year. The service provides varying levels of care depending on the needs of the individual. The Intensive home nursing service also offers a resource to other services to enhance and complement their care which may include:

  • A visiting service by two CSW up to four times a day to provide personal and emotional support.
  • A night service from 10pm until 7am.
  • Care up to 24 hours for those patients who are in the last few days of their life.
  • Support for relatives to attend appointments

We work closely with STH Community Nursing, Continuing Health Care (CHC) and the wider STH Trust.

To access this service you will need a referral from your GP or other healthcare professional involved in your care.

All health care professionals with access to this system should refer via the electronical referral form on Systm one.

Health care professionals who do not have access to systm one should refer via the guidelines below.
