We are an advisory service to health professionals and the general public.
We accept referrals from GPs, Consultants and other healthcare professionals through the single point of access service (SPA) referral system. We do not take self-referrals.
Once we receive your referral you will be contacted and an appointment will be offered to you at one of our community based clinics. We also have a team dedicated to visiting housebound patients at home.
