Community Therapy

The Integrated Care Team Therapy (ICTT) is a community rehabilitation service providing therapy for patients in their own home or care home to help them to improve or maintain their independence as long as possible.

We are a team of physiotherapists, occupational therapists and therapy assistants dealing with a wide range of conditions such as neurological, respiratory, orthopaedic and mobility issues including falls.  The service provides person centred care with a ‘What matters to you’ approach to support people to reach their own personal rehabilitation goals.

  • Anybody over the age of 16
  • Anyone who is housebound or whose rehabilitation is more appropriate to be carried out in their own home environment
  • Any person registered with a Sheffield GP

You will usually be referred by your GP or other healthcare professional.

Referrals are made via Single point of access (SPA).

When we receive a referral for you, we will text you to let you know that you will be receiving a call from us.

We will normally contact you/relative or carer by phone within 2 working days of receiving your referral to understand ‘What matters to you?’, what issues you are having and how we can help.

We will either book you into a face-to-face appointment or redirect you to another service if we have identified something that other teams can help you with.

The face-to-face appointment involves us visiting you in your own home to assess your needs and working with you to meet your rehabilitation goals.

We will endeavour to provide care which promotes a person's dignity and self-respect, regardless of racial group, origin or nationality, gender, disability, age, sexual orientation, political or religious beliefs.

We have a zero tolerance to all forms of abuse and decisive action will be taken to protect staff and patients.

We will ask you to put any animals, excluding assistance dogs, in a different room. We also ask that you provide our community staff with a smoke free environment during the visit. This is for the safety of staff.

What to expect at assessment?

We will arrange a morning or afternoon appointment with you, and you will be able to have relative or close friend or carer present.

If your first language is not English, we have access to interpreters to support you.

We will discuss any issues which you are having and assess your balance, muscle strength and everyday activities to devise a personalised programme for you.

We can provide suitable equipment to support you to remain as independent as possible.

How can I access the service?

A GP or health professional usually refer you.

When will I be seen?

We are a 7-day service, and our hours are 8.30am-6pm.

We will arrange a suitable am/pm appointment with you. These can be texted to you.
