The Community Nursing Service visits people in their own homes or in residential care homes and provides a range of care to support periods of illness and incapacity.
As well as providing direct patient care, we also help patients to understand their condition better, explain how they can help to improve their health and gain or maintain independence.
We will work with you to manage your health condition following an assessment and, in partnership with you and your carers, agree a care and/or treatment plan. We will discuss what matters to you and how we can help you.
Our service works within office hours and may visit you at any time during this period, unless otherwise required. We also offer a city wide service 24 hours a day when required.
You will be visited by a member of your local team, but this may not be the same person every time.
We will arrange a day to visit you. If you are unable to answer the door, we will need someone to let us in or access a key safe if you have one. All our staff are issued with staff identification badges.
If you need to cancel your appointment, please let us know as soon as possible. You will be advised of the correct contact number.
We will ask you to put any animals, excluding assistance dogs, in a different room. We also ask that you provide our community staff with a smoke free environment during the visit. This is for the safety of staff and to minimise risk of cross infection.
All staff will wear the appropriate personal protective equipment during their visit.
Clinical photography is a vital part of a patient’s clinical records, we will ask for consent prior to any photographs being taken. As part of your care/treatment, we may provide you with information and equipment. You will need to take responsibility for keeping these items safe. For equipment that is no longer required, you will be provided with contact details to arrange for it to be collected.
Please note: we are unable to collect prescriptions and supplies for you. Most community pharmacies provide a delivery service. If you would like to have a chaperone/interpreter present during the visit, please inform us prior to our visit.
To maintain your independence, we recommend that you also use your existing support networks. This might include family, friends or home-care. Community Services are able to offer further advice on ways to manage your specific health condition. If another more appropriate service is needed, we will ask your permission to pass on any relevant information to make sure your care progresses smoothly.
