Assessment and Rehabilitation Therapy team

The ART-Team provides a city-wide service and conducts a range of holistic assessment and slow-stream rehabilitation interventions for people with long term conditions. ART-T functions on a hybrid rehabilitation model which combines home visiting and the use of an outpatient gym and therapy room at Nether Edge Outpatient Clinic, Nether Edge Hospital, Lyndhurst Road.

ART-T supports people who have long term conditions including providing a specialist service for people with Parkinson’s. The team aims to improve/maintain a person’s ability to carry out normal daily activities as independently as possible.

Rehabilitation is provided by occupational therapists, physiotherapists, therapy assistants and speech and language therapists dependent upon the person’s goals.

The team also host a medically led falls clinic which provides an assessment of your abilities, falls risk and a medical review.  Following your assessment, we may recommend you access the rehabilitation offer.

Referral Criteria/Process – ARTT

Patients with a long-term condition requiring advice on prevention, staying healthy or rehabilitation.

Patients must have the potential for rehabilitation.

Referral Criteria/Process – Medically Led Falls Clinic

65 years and older with recurrent falls (or a single fall associated with significant injuries) AND observed to have balance and/or gait disturbance

50 – 64 years at higher risk of falls due to specific health factors

Medically unexplained falls (not slips, trips etc.) for example falls preceded by symptoms of dizziness, syncope, palpitations or vertigo 

Younger patients and those who have not had a fall but are at high risk will be considered for review on a case-by-case basis.

Patients with known established risk factors for falls are probably best seen by the respective specialist, e.g. PD, neurology, stroke, cardiology, ENT etc
