We are a community intermediate care service. This means that we can provide health and social care support to help you either come home from hospital (Discharge to Assess) or prevent you from being admitted to hospital due to an acute illness (Admission Avoidance), for example after a fall or a chest infection.
We are one of a group of services that provide an Urgent Community Response service (UCR), which is a national initiative to ensure anyone who is at risk of an admission to hospital receives an assessment for support within two or twenty-four hours as appropriate.
We work closely with and are able to make referrals to a range of community services, including Community Nursing, Community Advanced Clinical Practitioners, Therapists, Dieticians, Mental Health Nurses and Speech & Language Therapists.
Active Recovery is a NHS service and is therefore free at the point of delivery. If you require ongoing care or reablement the appropriate services within Sheffield City Council will arrange an assessment and if you have eligible care and support needs, you may need to contribute towards the cost of your care. Active Recovery only provides the minimum level of care that will keeps you safe at home and will be reduced, stopped or transferred when clinically appropriate.
