Being diagnosed with a heart condition

It's common to feel a variety of emotions after being diagnosed with a heart condition, especially if you have a sudden heart attack or need emergency heart surgery. People say they feel angry, shocked, confused, anxious, and often sad.

Living with a long-term lung condition

Living with a long-term condition such as COPD, asthma, fibrosis, or bronchiectasis can result in increased fear, anxiety, worry and panic.  These symptoms can also affect how you feel and can result in low mood, frustration, increased irritability and negative thinking.

We can help

Our service offers assessment and treatment for individuals who are experiencing mental health needs (eg anxiety, depression, panic) alongside and related to a cardiac or respiratory condition.  We offer treatment programmes to help improve an individual’s ability to manage periods of low mood and/or anxiety which can also help you manage your physical condition.

Individuals are usually seen in their own homes. Operating Hours 08.00 - 17.00 Monday to Friday

  • Referral Criteria
  • The referred individual must have a long term respiratory condition (eg COPD, pulmonary fibrosis, asthma) and has an identified co-existing mental health need.
  • Is over the age of 18
  • Ideally have completed a GAD7 and PHQ9 within the last 4 weeks
  • The referred individual must be registered with a Sheffield GP
  • The referred Individual is not actively receiving a service from an IAPT or Community Mental Health worker (although they can currently be under a secondary care mental health consultant).
