Active programmes (Cardiac and pulmonary rehabilitation)

Active Programmes is a Community based service made up of the following 3 teams:

  • Community Cardiac Rehabilitation Team, providing support and rehabilitation to people with cardiac conditions
  • Community Pulmonary Rehabilitation Team, offering a specially designed programme for people with chronic lung disease
  • Community Cardiac and Respiratory Mental Health Team, offering mental health support for people with cardiac or chronic lung conditions
