The central campus sites (Royal Hallamshire Hospital, Jessop Wing, Weston Park Cancer Centre & Cahrles Clifford Dental Hospital) are around a 25 minute walk from Sheffield City Centre. Please click here for a detailed site map.
The referring clinician should complete the relevant dental referral form. All sections of the forms must be completed; failure to complete all sections will result in the return of the referral and subsequent delay in patient care. Please complete all patient details including NHS number (if held). The file should then be saved using the following naming convention; forename, surname, DOB, date of referral and attached to an e-mail sent from an nhs.net e-mail address to the to the following e-mail address sht-tr.ccdhreferrals@nhs.net
The information on the forms should encapsulate the results of a referring practitioner’s examination and diagnosis.
To minimise duplication of work, any relevant test results, diagnostics and radiographs should be included with the referral.
Please ensure that radiographs should be sent electronically and as either JPEG or cicom files, not as a PDF attachment. Please include date of the images so that the files can be uplaoded to the patients clinical record.

There is parking available at the Hallamshire Hospital, Jessop Wing, Weston Park and Northern General Hospital, however at busy times spaces can be limited so please allow yourself extra time to find a parking space. We operate a pay & display parking system and prices can be found by clicking the link below.
We have shops and cafes for visitors and staff at the Northern General Hospital, the Royal Hallamshire Hospital, Jessop Wing and Weston Park Cancer Centre offering a wide range of hot and cold food, drinks and snacks. We also have vending machines at each site, which are available 24/7.

Please note that you may be greeted with 'Sheffield Teaching Hospitals' as opposed to the name of the hospital site. Please be assured that you are through to the right place and our operators will be able to direct your call to the correct hospital, person or service.
