Pre Travel Questionnaire

In order to ensure you receive accurate and up to date travel advice we require some pre travel information from you first. This will only take a few minutes and is a free service.

Once you have completed the questionnaire.

Please email the form to sheffieldtravelclinic@sth.nhs.uk 
and phone 0114 271 2608 to book an appointment. If you can’t email the questionnaire please bring it to your appointment.

A small charge of £15 is made for your initial appointment as this is a detailed consultation with a travel clinic specialist to identify all your travel health needs
 

At the time of your appointment

• Please bring any other paperwork you feel maybe important to your travel health consultation.
• The consultation may last up to 90 minutes depending on the complexity of the travel advice and vaccines you receive. 
• Payments will be taken using a debit/credit card.
