What happens if I do not find it easy to settle into adult services?

 

Firstly, your named worker will talk to you and the people that look after you about what you are finding difficult in adult services. They will do everything they can to make things easier for you. You can discuss any problems and how you feel with any member of the team.

Hopefully, as a team we will then be able to improve things here in adult services for you.

What happens when I think I have settled into adult services?

If you have needed some support to settle and are now feeling that you can manage more independently you can just let us know. We will check with the members of the team who have been involved in your care in case they are aware of anything else we could help with and of course you can always talk to us again!

If it is felt to be helpful an informal meeting with the transition team, you and any friends or family you want to invite can be arranged to make sure that you are happy to take on a more independent role.

The transitions team will still be there for you to contact in the future if you need.

How do I find out more about the Sheffield Kidney Institute’s transitions team?

You can ask any staff member here at the Sheffield Institute. They should be able to put you in touch with their ward, or department’s link worker from the transitions team.
