Do you know a healthcare hero?

Why not nominate them for an award?

Each year Sheffield Teaching Hospitals NHS Foundation Trust recognises dedicated staff and volunteers with a special awards ceremony.

The Trusts Thank You Awards, which has been running for 13 years, celebrates individuals and teams that go above and beyond the call of duty to care for patients.

This year the Trust is launching a new category called The Patient choice award which gives patients, visitors or members of the public the opportunity to nominate a staff member or team who they feel have made a difference either in hospital or the community.

The shortlisted nominees will then be profiled in the Sheffield Star and readers will be asked to vote for a winner.

If you would like to nominate a member of staff or volunteer from the hospital or community services, you can use our nomination form here, if you'd like further information about the awards, you can find more here: www.sth.nhs.uk/awards.

The closing date for nominations is 15th July 2016.

If your nomination is successful, you will be invited to attend the awards ceremony where the winners will be announced.

In order to nominate, you must know the full name of the nominee and the location where they work. If your nomination is successful, you will be asked to be briefly filmed for the award ceremony video, explaining why you have nominated the individual or team.

You must be aware that if the nominee doesn’t consent to take part when we contact them, we will not be able to process your nomination.

Sheffield Teaching Hospitals is one of the UK’s largest NHS Foundation Trusts and has over 16,000 staff caring for over a million patients each year. The trust cares for patients across five hospitals including, the Royal Hallamshire Hospital, the Northern General Hospital, Charles Clifford Dental Hospital, Weston Park Cancer Hospital, Jessop Wing Maternity Hospital and also in the local community.

The Thank You Awards are kindly sponsored by Sheffield Hospitals Charity which is the official NHS charity that helps patients being treated for almost every condition across all the Sheffield Teaching Hospitals sites.
